Medical Student Scholarship Application

Medical students entering their second, third, or fourth year of training in pursuit of a medical doctorate degree or its equivalent at the following Houston-area institutions:

Full-time students currently enrolled in Houston-area medical schools (listed above) who have completed their first year as of June 30, 2026 are eligible to apply.

• ✓ Student must be in good academic standing.
• ✓ Student must be an active member of the Student National Medical Association (SNMA).
• ✓ Previous HMF scholarship recipients are ineligible to apply.

  Completed Scholarship Application Submit the online application form (below) before the deadline.

  Official Medical School Transcript Must be sent directly from your institution via email — from the registrar's office or the institution's official transcript delivery service. Send to scholarships@houstonmedicalforum.org

  SNMA Membership Documentation Electronic receipt of dues payment, or a signed letter from the chapter president (signed PDF).

  Two (2) Letters of Reference One letter must be from a faculty member at the medical institution. Reference letters from family members will not be accepted. Letters of reference should be emailed to scholarships@houstonmedicalforum.org on or before Friday, July 31, 2026.

  FAFSA® Documentation (Optional — Financial Need) A completed Free Application for Federal Student Aid form, if you wish to be considered based on financial hardship.
